What to bring to your appointment
  1. Your current valid insurance card.
  2. Referral authorization from your primary care provider if necessary (example: workers compensation statement, HMO/PPO referrals, etc)
  3. Medical history including the names of all medications and supplements you are currently taking and their dosage
  4. All previous X-rays, CT Scans, MRI Scans, or other test results relating to your current medical problem.
  5. Previous medical records relating to current medical problems, including any prior surgery reports.

  7. Children under 18 must be accompanied by an adult.  If you are sending your child with someone else, a letter must accompany the child advising that the adult can represent you.  Patient authorization forms are located on the website to complete and sign in order to allow them to be brought in with the child.
