I will be closing my orthopaedic practice at Northwest Orthopaedic Associates & Sports Medicine effective June 24th, 2008.  I will not perform elective or emergent surgical procedures after March 26th, 2008.  I will be available for your urgent care until June 24th, 2008.

 

If you require ongoing orthopaedic care for problems which we have addressed together in the past, then I recommend you establish contact with your primary care physician, another orthopaedic surgeon here at Northwest Orthopaedic Associates or one of several other orthopaedic surgeons here in the Tri-Cities Community.

 

The names, addresses, and phone numbers of orthopaedic surgeons practicing in the Tri-Cities may be obtained by calling the Benton-Franklin Medical Society at (509) 582-2866.

 

If you are under a managed-care insurance situation, you may need to contact your health care insurer or primary care physician for a referral.

 

Contact Northwest Orthopadic & Sports Medicine for an authorization form to release a copy of your medical records.  On receipt of this signed form, my office will forward a copy of your medical records to you or to the physician you designate.  If you choose to stay with a provider at Northwest Orthopaedic no change of medical records is needed.
