Fees and Billing
All fees are due and payable at the time of service. As a courtesy to patients we bill most health insurance carriers, provided we have sufficient information. However, you retain financial responsibility for all charges incurred and will receive a monthly billing statement. We do not bill other insurance carriers (auto, home, etc.) and require payment at the time of your appointment for services provided under these plans.
Our physicians accept assignment on Medicare claims and are preferred providers with several insurance carriers.
Please notify our staff of changes to address, telephone number, marital status, insurance coverage, etc., which may have occurred since your last visit.
A nominal fee may be charged for each form that various insurance carriers, state departments, or employers require the physician to fill out.
